- He applied for US Passports while a resident of Riverside, California in 1917 and 1918. On these applications, his full name was shown as Herbert Sanford Maltby, born in Placerville, California on 14 Apr 1864. His father was listed as Isaac Maltby.
He indicated that he had previously lived outside the United States, in the Canal Zone for several days in November 1914. Both in 1917 and 1918, he was applying in order to serve as a Missionary in British India.
In 1919 he registered with the Consulate General of the US at Calcutta, India as an American Citizen. He was a resident of Benares, India doing Missionary Work on the behalf of Pilrim's Mission in Benares. He stated his legal residence was in Atlanta, Georgia.
In 1921, while a student in Fulton County, Georgia, he again applied for a passport, once again to serve as a Missionary in India.
He died in Melrose, South India, at the age of 73, in 1937. His wife was shown as Lillie Doll Maltby, also of Melrose. Cause of death was senile cerebral softening, hypostatic congestion of the lungs, and exhaustion.
